MODULE 1 LESSON: Accident Reporting and Investigation

We all know accidents happen. To us, and to anyone. Some accidents can lead to pain, and some can lead to serious injuries, or even death. So, it’s really important to understand why accidents happen. Unless we understand the “why” we’re almost guaranteed to make them again and again.

An accident can be caused by hazardous conditions or unsafe work practices. But, even when the reason seems obvious, the root cause can often be something else. This is where accident investigation comes in. Accident investigation is not fault finding. Its fact finding. It examines an incident systematically to determine its true causes.

These determinations and the information leading up to them are used to, in many cases, update policies, procedures or equipment to reduce the risk of that type of accident ever occurring again.

Again, the point of an accident investigation is not to assign blame or to get anyone in trouble. It’s  to prevent people from being injured in the future. But before we discus the investigation let’s talk for a minute how we should deal with the incidents themselves.
